Social workers face increasing pressure to provide outcome data to ensure high-quality client interactions and value to the palliative care process. Our practice emphasizes individualized assessment and planning, as well as interventions tailored to the person in the context of their lives and dynamic circumstances. Capturing meaningful outcome data based on palliative social work practice in palliative care has proven to be challenging. This is especially so when social workers provide service in a team context, and it can be difficult to tease apart the social work impact and dimensions of that care.
This webinar will describe a model for palliative social work practice evaluation based on single system design principles that can be easily adapted to this practice context, and yield outcome data that can be used to illustrate social work contributions to the team effort and client outcomes. Based on the principles of individualized problem identification and matching interventions to that issue, this method for evaluation matches social work practice demands well. It can be adapted for a wide variety of client situations, and encourage accountability for the palliative social worker in practice. This presentation will describe this model, demonstrate how to apply it to case scenarios, incorporate intervention approaches and social work theory, as well as including interactive sessions where participants can begin to plan and practice how to incorporate evaluation into their work.

About the Instructor
Stacy Remke, MSW, APHSW-C is a clinical social worker with over 25 years of experience as a pediatric palliative care social worker. She is currently an educator at the University of Minnesota School of Social Work, teaching in the areas of grief and loss, health care social work, and inter-professional practice. She has published on topics related to pediatric palliative care, team effectiveness, and palliative care program development. She teaches nationally and internationally providing palliative care training to interprofessional groups. Stacy is also a member of the Board of Directors of SWHPN.
